Model release

I've never uploaded anything which would need model release so I'm new to this. I found this model release from alamy https://www.alamy.com/contributors/alamy-model-release-form.pdf

Honestly, I don't understand how this works.
Does this has to be printed, signed, scanned/photographed and then uploaded again.
Does all releases requeire photo of a model (in my photos only hands are visible) Do I have to put photo of a model even if only hands are visible?
Do all releases require photo of a model?

Yes printed and scanned. Every agency has different standards Alamy are super strict even the tiniest body part  tends to need a release.

I have submitted a finger without any model release (not Alamy). And yes, normally it has to be printed, signed and scanned / photographed. Or you can use an app but check if the agency accept it
