Title: Model and Property releases

I have read on the dpreview forums about putting images on more than one agency as long as the images are not "exclusive rights".  I am wondering how to handle model and property releases.
As of now, I only work with Istock.  Their model and property release forms have their name on them.  Are there generic forms that all agencies will accept or must you get a signed release form for every agency you submit to???

No, using a generic form is fine, as long as it has all the information they require. I can see if I can find the one that I use, and upload it here.

Here I have included the release I use.

One important thing is that the release includes the models name and signature, and a witness name and signature.  The release must also include the ph #, address, and email of the release.  I have also put a place for the models picture, so that you can remember which release belongs to who if you have quite a few of them.

I am unable to download the generic release form graciously provided by Leaf.  Can anyone provide a copy or have another one they are willing to share?

after running into problems with istock with not having enough info from my models, I have started to use the istock release.

However dreamstime doesn't like the istock releaes (because it says istock on it)  Everywhere else accepts it mind you.

What do you all do?  Is it acceptable to just crop out the istock info?
