I am not sure if it is just me or not but I thought I would post this.  I tried to fill this out and immediately got a Malware Trojan that has taken my laptop completely down.  It has either removed or hopefully hidden all of my files.  Again, not sure if just me, but just a word of caution.

Alright, I think I have things cased.

Thanks for reporting the problem guys.  I have checked through the files and found one hacked file.  It was a typical WordPress type attack where they add a bit of code at the end of the index.php file.

Anyhow, I have since repaired that file and things should be looking good again.  Let me know if you see any problems.

Funny that AVG didn't spot that file when I was testing the survey  Hopefully not too many people were scared away.

No probs here either. Completed form also AVG.

Thanks for the update Noodles.  Yeah, maybe I'll at least remove the virus posts so we don't scare everyone away... if it is looking good now for everyone.
