The new interface at IS states "Only From Istock" for exclusive files. Is the purpose to inform buyers that you don't need to shop around to find this image cheaper elsewhere? Has that been a real problem?

My fear is that it will eventually mean to buyers that these images are gonna cost you a whole lot more.

I have often wondered how important it is to buyers that a certain image can only be found here at iStockphoto. As a buyer myself, it does not mean much.

It's not even true. Yuri and some other pseudo or quasi exclusives have their photos elsewhere, even the same ones.

I'd have thought it would benefit buyers who are time-conscious, as it should mean that there is no point in searching for the images elsewhere.
It is perceived as a flag meaning 'more expensive', and as they have chosen not to help buyers by having a $ indication on each file (s they used to), that means that even exclusive files demoted to Main suffer by being perceived as being 'more expensive'. So even exclusive 'Main' files can't compete fairly with indie files (other than the indication that you shouldn't be able to find them elsewhere).

Price conscious buyers can filter on price, but when I'm shopping I like to see everything so that I can see that X is , Y is and Z is and make a decision on that - I don't immediately head for Poundland.

Some even have their own "Vetta" (tm) collection offsite: http://peopleimages.com/support/prices

Our vetta collection contains images that tell a story about a slice of life, images that make you stop, images that make you smile, laugh, cry or relive. Images about what it is to be human, the things we love, the things that matters, the moments we remember and the dreams that we aspire. The higher price is actually perfect for the sophisticated clients, because images in this collection are downloaded less than 1% of our main collection and are an excellent choices for projects that are high profile and where you don't want the image to be used many times by other designers.

Isolated models...

I think the label is a double-edged sword. People can say "no use searching for this image elsewhere," or people can say "I'll scroll down and look at the images that are not available only at iStock, because I can find those for less elsewhere."

Not a lot really.  On the one hand there is quite an amount of stuff labelled "only from istock" available elsewhere and, on the other hand, stuff truly only from istock but not labelled as such, because no other site, with the possible exception of flickr, would accept it.

What does "Only from iStock" mean?

It is very simple:
Maybe you have noticed that now iStock accepts anything.
So they accept any images refused by any other sites.
These images are "Only from iStock"

Little late to this thread but yuri uses the TM word Vetta on his personal site!! Jeez, I guess the rich are different.
