Problem filling out form W-8BEN

I have some problems filling out form W-8BEN, i'm from Venezuela, individual. I look for the Internet web page address, but it told me error 404. Someone could help me with this, please? Thanks you!

of the treaty identified on line 9 above to claim a _______% rate of withholding on (specify type of income): _____________________________________"

I don't know, why when a choose Venezuela as country (country i'm living) , the form, fill Venezuela on line 9, for that reason the form pushing me to fill the line 10.

Well, i think i solve this. I use Google and find this.
