Hmmm... is this a double post, I tried to insert a Word file and it wouldn't let me post, so I copied here and when I hit post the message said you already posted this... couldn't see it so trying to remember what I said.

This was for some kids to make prints and post to Facebook.  It is more wordy than most, but I didn't want them to post full rez photos and I didn't want them using the photos in their marketing for upcoming events.   I hope this helps.

Grant of Limited Photographic Usage Rights

I, Lorraine Swanson, do hereby declare and warrant that I, the copyright owner of these photos, have given permission and license to you (Parent) (hereafter  the �Customer�) to submit for processing or reproducing dance photos taken by me on October 24, 2010 at ________________ of the model  _________________________ and I represent that I have a legal right to grant such a license.

Customer is not permitted to post full resolution photographs to the Internet; however, Customer has been provided with a folder of web sized images that visibly contain my copyright notice which may be shared on the Internet or shared electronically. 

Wherever reasonably possible, please attribute credit as �Photograph �2010 Lorraine Swanson�

ITPC data must not be stripped from the photographs.

Customer does not have permission to make copies for sale or for the purpose of circumventing sale of images by the photographer.  Customer may not authorize any reproduction of the images for commercial use.  Customer is personally responsible for communicating the limitations of this contract to any person who receives a copy of this disc. 

By way of example, Customer may print copies for personal use, portfolio, or to share with friends and family.  Customer may provide photos to a publisher for use as editorial content as it pertains to the photograph or person in the photograph, and it may be used as a profile photo on a print article or as an avatar on a social networking site.   Photograph may not be incorporated in any commercial material, or as part of an advertisement, or placed on a product that will be sold, or used to sell another product.
