can someone direct me to a standard model release that will be acceptable across all agencies?  Thanks!

I have the most success with the canstock release form.

many people have success with the Getty form.

I recommend the Getty form and the Shutterstock form (use both).

Sorry to hijack your thread, but I think its  better if I dont create a new thread. I have pictures of my wife which were taken in different days, can I submit universal model release for all days or she needs to sign model release for all days, I want the pictures to be accepted at shutterstock

Yes you can, I don't think they bother much, I've use the same release for my girlfriend a month apart, different location and different clothes, no problem, how could they tell if the shoot were taken a few months or few minutes apart.
Though I wouldn't do that for other models outside close friends and family, for own protection.

they can if the shoot location address is kinda noticeably different. For instance, you shoot in the beginning of the month on the forest, and then with the same model, you shoot in the following week inside of an apartment. Different locations, different addresses, different model releases. ;)

It's advisable to have a release for each shoot.  The EXIF data in the image files contain the dates they were shot, assuming of course you set it correctly in your camera (and haven't modified it later).  Most of the agencies (all but iStock?) require only a single release for each model to be uploaded, but having the paperwork could save you a lot of legal trouble down the line.
