Must I fill in the form if currently i am holding 28% tax and my country has no tax treaty with US?

Dear All

I am having problem with Tax Form Apply in Fotolio.com, I am not sure on how to fill in the form, My country in not have a tax treaty with US.
currently all my earning is holding 28 % of US tax, I am living in Cambodia. i would be appreciate if i can see you guy form filled for the same country that has no tax treaty with US.
