Title: Video model release accepted by all?

Wonder if anyone can point me to a model release form for video, which will be accepted by all agencies. Thanks!

I just used the photo release I had and added 'and video' anywhere I saw photo.  so.. The model agrees to appear in photos and video... or ... any photo and video can be edited, altered etc.
