I can see that the reason for the rejection was that it's not written the relation of the "Parent/Guardian" and the child (father, mother, sister, etc.).
The Content team has increased the security level for new uploaded files and now the Curators also need to see that described information, which is the relation between the minor and the person signing as "Parent/guardian".
Please, add the info to that same model release that you have already and upload it again to your Pond5 account. If you need assistance with the upload, you can send it to us and we'll upload it from here.
