It�s been a long time since my last upload.
I used to use one model release in english and spanish (everythinig in the same page) and it used to work but I wonder which one are you using now for all agencies (if possible) and if there are any changes in the release acceptance by the agencies...

iStock works well for me, for all agencies. But you have to fill one for each date.

I use the app releases, you can pick from many forms. I mostly use shutterstocks.
