Good question and you will probably get frustrated from many opinions.  I gave up, choosing instead to let the agencies handle that part.

A google search returned many options ... mostly taking something from another agency and adapting it to your specific needs.  Rewriting a template to serve your specific purposes would likely involve hiring legal assistance;  particularly a copyright lawyer.

My opinion.  I'm sure some of the more experienced (business savvy) photographers have worked this out.  Good Luck.

If you use the template for one client only, of course it's allowed under normal RF. If you"re going to resell (or give away) the template for general use, then and only then you'll need an extended license.

I'd suggest that you make sure you have a period of time limit and that it's only for one use, clearly stated in the contract. You can probably write your own and keep in mind that a simple contract is easier to enforce than a complicated one. It's all legal boilerplate in the ones the sites use, but someone like an individual could easily says, in plain English, that you are licensing the photo for one time use, for non-commercial purposes (not for resale), for a period of one year. Addition uses will require another license fee.
