In the generic model release(istock) there is 'Attach Visual Reference of Model' section.

Is it ok to put over the model picture on the scanned release jpg file in photoshop?

or do i have to print the model picture and attach on the paper release, and then scan it?

me too.  I just take a headshot from the shoot and layer it on top in photoshop.

what i usually do, like many other photographers, is have the model pose with the signed MR.
it clears up future incidents like, "oh, i  never gave you permission";
something you won't get with pro models, but the wannabee poodle posing prima donna do pop up
to surprise you with that. thankfully not too often.

Like etudiante_rapide, I also take a photo of the model holding the release.  I use that photo as the Reference photo.
